Supporting good assessment: Practice Tool (2014)
Assessment is a central part of adult social care, both as a gateway to support and as an intervention in its own right.
These tools support organisations to deliver good assessment, develop good assessors, ensure good support for assessment and think through the implications of delegating assessments to other parties. The tools reflect existing good practice that is consolidated by the Care Act 2014. They can be used by organisations to review and strengthen their assessment delivery.
